Экстренное и плановое хирургическое лечение новообразований придатков матки

Цель исследования. Обсудить клинические аспекты дифференциальной диагностики и лечебной тактики в отношении пациенток с новообразованиями яичников, показания к экстренным и плановым хирургическим вмешательствам.
Материал и методы. В обзор включены данные зарубежных и отечественных статей, найденных в международных базах данных по данной теме, опубликованных за последние 20 лет.
Результаты. Наиболее значимыми факторами риска развития рака яичников являются семейный анамнез, возраст, использование заместительной гормональной терапии, эндометриоз и ожирение. Дифференциальная диагностика основывается на данных трансвагинального ультразвукового исследования с учетом диагностических критериев IOTA,определение уровня маркеров CA 125, HE4, ЛДГ, ХГЧ, АФП. Оптимальным методом хирургического вмешательства является лапароскопическая операция с интраоперационным и плановым гистологическим исследованием.
Заключение. При постановке диагноза злокачественной опухоли яичников лечение должно проводиться только в условиях специализированных клиник.
